Care Planning & Quality Assurance Officer

About the role

The Care Plan & Quality Assurance Officer is responsible for developing care plans from the ground up, completing comprehensive risk assessments, maintaining accurate records within the Access system, and conducting structured quality assurance calls with service users. This role works closely with the Registered Manager to ensure care plans and risk assessments are accurate, up to date, and compliant with regulatory standards. All risk assessments completed by the post holder will be reviewed weekly by management.

What you'll do

  • Care Plan Development
  • Develop comprehensive, person-centered care plans based on initial assessments and referral information
  • Ensure care plans reflect each individual's needs, preferences, goals, cultural considerations, and communication requirements
  • Collaborate with the Registered Manager to review, finalize, and approve care plans
  • Update care plans promptly when service user needs change
  • Ensure consent documentation is properly completed and recorded
  • Risk Assessment Management
  • Conduct detailed risk assessments including falls, moving and handling, nutrition, skin integrity, environmental, and behavioral risks
  • Ensure risk controls are clearly aligned with care plan interventions
  • Submit completed risk assessments for weekly management review
  • Escalate high-risk concerns immediately when identified
  • Documentation & System Management
  • Maintain 100% accurate electronic records within the Access care management system
  • Monitor review schedules and ensure records are updated on time
  • Conduct monthly file audits to maintain documentation quality
  • Ensure full compliance with data protection and confidentiality standards
  • Service User Quality Assurance
  • Conduct structured quality assurance calls with service users and/or their representatives
  • Accurately record feedback within the system
  • Identify patterns or concerns and escalate them to management
  • Produce service user satisfaction summary reports every two weeks
  • Reporting & Governance
  • Provide weekly updates and reports to the Registered Manager
  • Support preparation for CQC inspections and regulatory reviews
  • Maintain documentation that is audit-ready at all times
  • Essential Qualifications
  • Registered Nurse (RGN, RMN, or RNLD) or Degree in Health & Social Care, Social Work, Public Health, or a related field
  • Evidence of ongoing professional development

Requirements

  • Minimum 3 years' experience in health or social care
  • Experience developing care plans from scratch
  • Experience conducting comprehensive risk assessments
  • Experience working in a highly regulated environment
  • Experience using electronic care management systems
